Based on a comparative analysis of agencies involved in Fazzi Associates’ Home Health CAHPS system for 2019, HCMC Home Health & Hospice’s results for the Overall Satisfaction or Likelihood of Recommending survey questions placed it in the top 25 percent of Fazzi’s National Home Health CAHPS database.
The HHCAHPS and Hospice CAHPS Top 25 award is presented annually to organizations that subscribe to Fazzi’s Patient and Family Satisfaction Services and have demonstrated superior performance in overall satisfaction results.

Home Health is designed to provide the smoothest transition from hospital to home. Patients must be referred to Home Health and must be homebound, need skilled care in the home and be under the care of a physician. Hospice is support for people with limited life expectancy and their families. We provide a comfortable, supportive environment, ensuring that the quality of life is the best it can be. We pay special attention to physical, spiritual and psychological needs of our patients and their families. Hospice patients must be certified by their primary care physician as having limited life expectancy and their physician must agree to manage their care. Referrals to hospice are made by physicians, hospital personnel, patients, family, friends, and clergy.
