Cardiac Rehabilitation
The Cardiac Rehabilitation Outpatient Program, Phase II, is an exercise and education program designed to help you take better care of your heart. The exercise program will be designed just for you by your physician, with the help of a registered nurse, to help your heart become a stronger and more effective pump. You will be closely monitored during the sessions to evaluate your response to the prescribed exercise. Your physician will receive reports on your progress.
Who Needs Cardiac Rehabilitation?
You can benefit from Cardiac Rehabilitation if:
- You have had a Myocardial Infarction (MI) within the past 12 months
- You have had Coronary Artery Bypass Graft (CABG)
- You have stable angina with current documentation in your medical chart
- You have had a heart transplant
- You have had a valve replacement/repair within the past 12 months
- You have had coronary stents within the past 12 months
- You have Heart Failure with ejection fraction less than 35

- Cigarette Smoking
- High Cholesterol
- Inactivity
- Diabetes
- Obesity
- Stress

What does it cost?
Cardiac Rehabilitation is reimbursed by Medicare and most other third party insurance carriers.
Who will be working with me?
Exercise sessions are telemetry monitored and you will be under the supervision of registered nurses who are certified in Advanced Cardiac Life Support.
The nurses will help you through each exercise session, talk with you about your concerns and keep you and your doctor informed about your individual progress.
How do I get started?
After your physician prescribes Cardiac Rehabilitation, you can then be scheduled for an evaluation.
Exercise sessions are three times a week and the nurse will appoint times that are convenient for you. It is typically a 12-week program on Monday, Wednesday and Friday for approximately one hour each day.
And after 12 weeks?
After 12 weeks, an optional Phase III program is available. Phase III costs $35 per month and must be pre-paid at the beginning of each month. Ask your cardiac rehab nurse for more information.
